To make an appointment please call us on
(02) 9650 4422 and advise us of your preferred consulting room location.
Once you have made your first appointment, you will need to fill out a new patient details form.
This will enable us to process the information in preparation for your appointment.
Please remember to bring tHE following items to your consultation:
- A referral from another doctor. A referral from a GP is valid for 12 months, whereas a referral from another specialist is valid for 3 months. A referral is needed so that you can claim the Medicare rebate on the consultation fee (this is a Medicare requirement).
- Medicare Card and Health Fund details (where applicable).
- Any previous investigations such as ultrasounds or x-rays
- Payment for the consultation fee. Payment is required at the time of the consultation. We accept Visa, MasterCard, eftpos, cash or cheque. We do not accept American Express or Diners.
Cancellations
Please telephone the office during business hours or leave a message on our voice mail. We would appreciate it if could have at least 2 days notice for a cancellation so that your appointment time can be allocated to another patient.
If you decide not to go ahead with a surgical procedure that has been booked please call us as soon as possible so that the time can be allocated to another family.
Whilst we make every effort to run on time emergencies can occur that can cause delays beyond our control. This may involve rescheduling you appointment at short notice. We apologize in advance if you are kept waiting.
Emergencies
If you have an emergency during office hours, please call the office on 9650 4422 and we can advise you on a course of action.
Outside business hours, please call Sydney Children's Hospital on 9382 1111 and ask for the surgical registrar on call.
Alternatively, you can present at the Emergency Department of Sydney Children's Hospital where you will be seen by the surgical registrar who will advise you on the most appropriate course of action and contact Dr Henry if necessary.
